Insurance
Carlock Counseling does not process insurance, but we can provide a form for you to self-submit to your provider for reimbursment. If you need to use insurance, call your provider and ask about the process of self-submitting claims and reimbursement. You might consider asking them the following questions:
- Do I have mental health benefits?
- What is my deductible, and has it been met?
- My counselor does not process insurance. What is the process of submitting claims myself?
- Do you cover marriage counseling?
- How much does my plan cover for an out-of-network provider?
- Is preauthorization required?
Payment
Payment is due in full at each session. Payment for all services is accepted in the form of cash, check, Health Savings Account (HSA), or any major credit cards. There is a $35.00 fee for a returned check.
Cancellations/Rescheduling Policy
We set appointments based on your requests and our availability as your provider. If you need to change your scheduled appointment, we need as much notice as possible to accommodate the change. We understand that emergencies do come up; however, if you fail to cancel or fail to show up to your appointment with at least 24 hour advanced notice to us, we reserve the right to charge you a $50.00 cancellation fee since it is reasonably likely we will be unable to offer your appointment slot to another client. Please note: Cancellation fees will be your responsibility to pay in full upon receipt of an invoice from us.
On occasion, it may be necessary for our provider to cancel an appointment that has been scheduled with you. Should this need arise, we will make every effort to communicate this fact to you using your contact information that we have on file as early as possible in order to avoid your making a special trip to our office and to reschedule your appointment with your provider. We are not responsible for our inability to reach you in advance of your scheduled appointment, nor are we responsible for any form of cancellation fee where we cancel the appointment for any reason.
